What is Toxic Epidermal Necrolysis (TEN)?

Most experts agree that Toxic Epidermal Necrolysis, or TEN, is a more life threatening manifestation of Stevens-Johnson Syndrome (SJS), in which a larger percentage of the body experiences lesions and skin loss.

The classification for SJS and TEN is typically:

  • SJS – 10% or less of the body is involved.  At this level, the morbidity rate (rate of death) is usually 1% - 5%.
  • SJS/TEN overlap – 10% to 30% of the body is affected.  Morbidity rate at this level is 5% - 15%.
  • TEN – More than 30% of the body is affected, usually involving serious lesions and scarring in the lungs that interfere with breathing.  Morbidity rate for TEN at this level is as high as 40%.

Symptoms of TEN

As with SJS, TEN disease starts with a rash that quickly and aggressively spreads.  In as little as a few minutes or as long as a few days after taking the medication that causes the reaction, blisters and lesions break out on the skin, usually starting on the face and spreading to the trunk.  The skin on the blisters is easy to rub off and the sores often look like a bull’s eye or target.

In toxic epidermal necrolysis, the damage spreads to mucous membranes such as the eyes and nose, genital area, and internally in the esophagus, lungs, and vagina.  Blindness, the inability to eat or drink, and difficulty breathing can all occur.  The rash and sores can continue to break out in clusters for two to three weeks after exposure to the substance causing the reaction.

Secondary complications such as dehydration, infection and organ failure are even more significant with TEN than they are with SJS.  Death occurs at a higher rate.

What Causes TEN?

The only established cause of SJS and TEN is medication.  Non Steroidal Anti-Inflammatory drugs (NSAIDs) such as Ibuprofen, have been shown to significantly increase the risk and cause SJS and TEN.  Ibuprofen is the active ingredient in many prescription and over the counter medications used to relieve pain and fever.  It is also an active ingredient in some over the counter cold medicines.
